If your portal is running on Windows Vista, Windows 7 or Windows 2008 Server and Asp.Net you do not need to make any changes to Internet Information Server 7.0 or 7.5 configuration if the web site application pool is running in "integrated pipeline" mode.
To use extension other than standard .aspx or no extension at all simply configure the module to use the extension that you want and you don't have to enable the "Run All Managed Modules For All Requests" option.
